Decisions regarding the care of a loved one are dif­ficult to make. Sometimes it is helpful to talk to a caring and experienced Adult Day Services professional who is knowledgeable about the aspects of Adult Day Centers.

Eldercare is a community based program for adults which encourages independence and sup­ports older adults in remaining active and healthy in their daily life while remaining in their home. The community is operated and managed via a contract with Lebanon County A.A.A. by Avenues.  Avenues is a multi faceted Human Service agency providing innovative services to persons with disabilities and their families since 1952.  

There are three main components of our day program. The program is open year round, Monday through Friday, 7:30 a.m. to 5:00 p.m. except certain holidays.

1. Quality personal care in a safe, supportive environment
2. Therapeutic Recreational Activities
3. Socialization and Independence  

Eldercare strives to preserve the dignity and enhance the quality of life for those we serve through a caring, safe, stimulating, therapeu­tic and enjoyable environment. We help en­able seniors to remain at home in the commu­nity for as long as possible by providing relief for caregivers and on-going support for the individual and their family.  

Eldercare is a cost effective, alterative to Long Term Care. It emphasizes an individual's capabilities by encouraging participation in therapeutic activities. It enhances an indi­vidual's life and enables them to live as independently as possible well into their golden years.

We are sincerely and genuinely committed to providing quality care for your loved one's needs.

Transportation can be provided by the family or by County of Lebanon Transit (COLT)

We individualize each plan to accommodate the wishes of the loved one and/or family.

Admission Process

1. Contact the Lebanon County Area Agency on Aging for an assessment
2. Contact the centers for additional informa­tion 
3. Make an appointment with your doctor to have the Center's Health Appraisal completed which includes a Mantoux Test. 
4. Contact the centers to set up days of atten­dance, transportation arrangements and complete all necessary paperwork.
