APPRISE: is a free health insurance counseling program designed to help senior citizens with Medicare. APPRISE counselors are specially trained volunteers who can answer your questions about Medicare and provide objective, easy-to-understand information about health insurance. Specifically, APPRISE can answer questions about Medicare, Medicare Supplemental Insurance, Medicaid, and Long-Term Care Insurance. APPRISE counseling services are available throughout Lebanon County. Some of the services included in the APPRISE counseling program are: a meeting held the first Wednesday of each month at the 7th & Maple Street Lebanon Senior Center describing Medicare, how it works, and possible future changes. Also consumers with Medicare questions can call 273-9262 to set up an appointment with volunteers to answer individual questions on insurance, or short questions may be answered by phone.
Attendant Care: Only for transitioned under 60 consumers. Very intensive “hands on” Personal Care.
Community Health Council: Non-profit organization that was created over 10 years ago to fulfill unmet needs in the County (50+ Festival, Under 21 Expo and C & Y Youth Fishing Day).
Contingent Liability: Funds owed to the AAA by the County for excess payments such as health insurance and workman’s comp.
County Funds: County Appropriation from General Fund
Department of Ed Meal Reimbursement: These are meals provided for breakfast, lunch and snacks at the two (2) Adult Day Care sites.
Direct Care Worker: Funding to eligible provider Agencies who provide in-home services. This funding can be used for employee recruitment & retention and education expenses.
Enhanced Nursing Home Transition: Same as NHT but involves consumers who have been admitted within the last 30 days.
Federal NSIP: Funding is based on the amount of meals served during the previous fiscal year (home delivered and center meals).
Federal, State and Local Program Income: Donations received through programs provided through Regular Aging Block Grant funds. The funds are allowed to be carried over and used as needed.
Health Promotion: Ongoing activities for older adults intended to help them maintain their physical and mental health. The program also focuses on disease prevention activities. Programs are offered at six senior community centers, senior housing facilities and the 50+ Festival. Some of the programs currently being offered through Lebanon County AAA are aerobics, yoga, Golden Sneakers Walking club, line dancing, shuffleboard, bowling, and swimming at the YMCA (September through May). Educational programs include information regarding prevention of falls, diabetes, stroke, arthritis, and proper nutrition. A video library is available for interested individuals and/or groups who wish to borrow materials on health related topics. A wide variety of brochures, pamphlets and booklets are also available.
Healthy Steps for Older Adults: HSOA program consists of two 2-hour workshops designed to provide information about health care, exercise, home safety resources and other available resources to help older Pennsylvanians prevent falls.
Human Services Developmental Fund and Community Services Block Grant: Funding received through the County CAP Agency to pay for the unfunded portion of MH or MR consumers in the Adult Day Care program. CSBG used for eligible consumers in personal care home support and adult day care.
Lebanon County MH/MR: Partial funding for MH/MR consumers in the Adult Day Care program.
Medical Assistance Outside Billings: The Agency acts as a broker for services provided to waiver consumers.
Medication Management: is a program designed to help older adults use medications appropriately. Educational programs are offered to groups at our six senior community centers, retirement homes, and at various senior group meetings. These are often presented by a licensed Pharmacist who reviews each person's prescription medications alerting them to any possible problems. Educational pamphlets and refrigerator magnets for holding individual medication information are available at the Lebanon County AAA.
Nursing Home Transition: Nursing Home Transition is a program to assist nursing home residents to return to the community and receive heavy in-home services through a Medicaid waiver program. NHT coordinator assists the nursing home resident with acquiring, setting up, and furnishing an apartment, as well as setting up necessary services to help them remain in the Community.
Ombudsman: Program to investigate nursing home, personal care home, or adult living center complaints or concerns within the facilities. The Ombudsman acts as an impartial mediator with facility administration to rectify resident or family concerns.
Options Cost Sharing: These are the fees that are generated from services (personal care, home support and adult day care) provided to consumers based on consumer’s income eligibility.
Out of County: Consumer in Adult Day Care program who live outside the County.
PDA Waiver: Funds used to pay for casework time involved with medical assistance consumers. This does not include in-home services provided that are billed through medical assistance. Services are funded through a special waiver of certain Medicaid restrictions, allowing payments typically used for nursing home care to be used for in-home services.
Pre Admission Assessment: A comprehensive needs assessment to determine the consumer’s level of care. It is provided for individuals needing medical assistance for nursing home care and for individuals who qualify for the boarding home supplement.
Regular Block Grant: This grant supports Administration, Home Delivered & Center Meals, Socialization/Recreation, Volunteer Services, Shared Ride Legal Assistance, Ombudsman, Information & Referral, Personal Care, Environmental Modifications, Medical Equipment & Supplies, Home Support, Adult Day Care, Casework Assessment & Care Management, Protective Services, and Consumer Reimbursement.
Special Projects: These funds (e.g. lottery) are intended for one-time expenditures for services, supplies and/or infrastructure enhancements that will not increase the consumer base.
State/Federal FCSP: The Family Caregiver Support Program's purpose is to reinforce the care being provided to the functionally dependent older adult relative alone in their home (Federal), with a caregiver living nearby, or with both the functionally dependent older adult relative and caregiver living in the same residence (State). Depending on the total gross household income for the prior year, family caregivers may receive up to $200.00 a month to help purchase services such as respite or in-home services; or to purchase care giving supplies. An important feature of the FCSP Program is that it allows the Caregiver to choose the services most needed to help care for the frail older relative at home.
Stimulus Funds: American Recovery and Re-Investment Act signed into Law in February 2009. To be used over the next 24-27 months for both Meals on Wheels and Congregate Meals at the (6) Senior Centers.
VA Medical Center: Consumers in the Adult Day Care program sponsored by the VA.
