Adult Day Care: Provide a protected setting for consumers who are not capable of full time independent living.

Attendant Care: Only for transitioned, under 60, consumers.  Very intensive, “hands on” Personal Care.

Congregate Meals & Socialization/Recreation: to provide meals & activities in a group setting @ one of the six Senior Centers.

Employment: Referral to Experience Works for potential employment opportunities for persons 55 and older.

Environmental Modifications: To adapt homes (with ramps, stair glides, hand rails, etc.) of eligible consumers, in order to prevent inappropriate nursing home placement.

Farmers Market Nutrition Program: To provide to seniors (60 and older), who are income eligible, resources in the form of fresh, nutritious, unprepared fruits and vegetables from farmers’ markets and to expand the awareness and use of farmers’ markets by providing four $5 checks to be used between June 1st and November 30th.

Home Health: Physician-ordered care supervised by RN.

Home Support: Light housekeeping

Information & Referral: To provide information about aging services and facilities to the 60 and older population.

Legal: To contract with Mid Penn Legal to provide services to eligible consumers for major financial, medical, or imminent eviction problems.

Meals on Wheels (MOW): Provide a hot and/or cold, nutritious meal to eligible home bound consumers.

Medical Equipment, Adaptive Devices: Purchase of durable and non-durable medical equipment. This includes Personal Emergency Response Systems (PERS), i.e. Lifeline.

 Money Follows the Person: Consumer has resided in the nursing facility or State Hospital for at least 6 months or more prior to discharge. They must be eligible for Medical Assistance (MA) 30 days prior to discharge. date. The consumer goes on MA Waiver, and all cost for services or supplies is paid by the Federal Government for the first year.

Nursing Facility Clinically Eligible (NFCE): Nursing Facility Clinically Eligible includes an individual who has an illness, injury, disability or medical condition diagnosed by a physician; and as a result of the diagnosed illness, injury, disability or medical condition, the individual requires care and services above the level of room and board; and a physician certifies that the individual is NFCE; and the care and services are either:

Skilled nursing or rehabilitation services as specified by the Medicare Program.

Health-related and care services that may not be as inherently complex as skilled nursing or rehabilitation services but which are needed and provided on a regular basis in the context of a planned program of health care and management and were previously available only through institutional facilities.

Nursing Home Transition: Taking consumers out of nursing homes and placing them back in the community with traditional aging services (waiver, options, family, etc.).

Ombudsman: To provide an advocate and protect consumers in long-term care facilities (such as group and nursing homes).

Personal Assistance Service: Similar to Personal Care, but more intensive.

Personal Care: In-home care for eligible, home bound consumers.  May include bathing, dressing/undressing, feeding, toileting, grooming, transfer and assistance with medications.

Protective: To investigate and act on alleged elderly consumer abuse issues.

Transportation: To contract with COLT in providing shared ride and center trip transportation services.

Volunteers: To recruit people of any ages to provide volunteers for programs such as APPRISE, Meals on Wheels, Senior Centers or Adult Day Care.
